Abstract Cervical pain is one of the most common musculoskeletal disorders experienced by workers with non-ergonomic working postures, including sugar palm farmers. Repetitive work activities involving prolonged neck flexion, extension, bending, and manual load handling may increase the risk of developing cervical pain complaints. This study aimed to determine the relationship between work posture and cervical pain complaints among sugar palm farmers in Rambai Village, STM Hilir District, in 2026. This study employed a quantitative cross-sectional design. The study population consisted of 32 sugar palm farmers, and all participants were selected using the total sampling technique. Work posture was assessed using the Rapid Entire Body Assessment (REBA) method, while cervical pain complaints were measured using the Nordic Body Map (NBM) questionnaire. Data were analyzed using univariate and bivariate analyses with the Chi-Square test. Since one cell had an expected count of less than five, the final interpretation was based on Fisher’s Exact Test. The results showed that 20 respondents (62.5%) had high-risk work posture requiring immediate improvement, while 22 respondents (68.8%) experienced severe cervical pain complaints. Fisher’s Exact Test produced a p-value of 0.050 (p ≤ 0.05), indicating a statistically significant relationship between work posture and cervical pain complaints among sugar palm farmers in Rambai Village, STM Hilir District. The Prevalence Ratio (PR) was 0.185 with a 95% Confidence Interval (CI) of 0.012–1.032. It can be concluded that there is a significant relationship between work posture and cervical pain complaints among sugar palm farmers. Therefore, farmers are encouraged to implement ergonomic principles, perform regular stretching exercises, take adequate rest breaks, and improve their working posture to reduce the risk of cervical pain.
