| Abstract | The tiered referral system is a healthcare delivery mechanism that regulates patient flow from lower-to higher-level health facilities in stages, based on medical necessity. However, in practice, the referral system at Hidayah Deli Tua General Hospital faces various obstacles, such as frequent disruptions to the digital referral network that hinder data verification, discrepancies between initial diagnoses from primary care facilities and subsequent assessments and a lack of specific medical equipment that could increase the risk of patient condition deterioration. This study is aimed to determine the influence of administrative and clinical factors on obstacles in the referral process for Social Guarante Health Administration (in indonesia BPJS) patients. A quantitative research method was used, utilizing a survey design and cross-sectional approach, conducted in March 2026 at Hidayah Deli Tua General Hospital. The study population consisted of all Sosial Guarante Health Administration patients undergoing the referral process, with a sample of 83 respondents selected via accidental sampling. Data were collected through questionnaires and analyzed using univariate and bivariate analysis (chi-square test). Univariate analysis results indicated that the majority of respondents rated administrative factors (61.4%) and clinical factors (51.8%) as poor, with 57.8% of respondents reporting obstacles in the tiered referral process. Bivariate test results revealed a significant relationship between administrative factors (p-value = 0.012 < 0.05) and clinical factors (p-value = 0.045 < 0.05) and obstacles in the tiered referral system. In conclusion, both administrative and clinical factors are significant determinants of referral obstacles at Hidayah Deli Tua General Hospital. The hospital is advised to improve its digital administrative systems and the quality of clinical services through healthcare worker training, as well as to provide a more stable internet connection to minimize obstacles in the referral process. | 1 |
