| Abstract | Background: Services in the Intensive Care Unit (ICU) demand fast, precise, and integrative actions. The success of this service is highly influenced by the head nurse's managerial function in directing staff nurses, who mostly have relatively short work experience (1–2 years) in a high-pressure work environment. Objective: To analyze head nurse leadership methods (authoritarian, democratic, transformational, and transactional) and their correlation with staff nurse performance in the ICU of RSU Sembiring Deli Tua in 2026. Methods: This was a quantitative study using a correlational analytic survey design with a cross-sectional approach. The study population consisted of 38 ICU nurses, and samples were selected using the Total Sampling technique. Data were collected through Likert-scale questionnaires and analyzed using the Spearman Rank correlation test (Spearman Rho). Results: Univariate analysis (mean values) indicated that democratic leadership (13.42) was the most frequently perceived style, followed by transformational (13.26), authoritarian (13.05), and transactional (12.95). Bivariate analysis demonstrated that all four leadership styles had a significant positive correlation with nurse performance (p=0.001<0.05). Transactional leadership showed the strongest/most dominant correlation ($r = 0.717), followed by transformational (r = 0.677), democratic (r = 0.624), and authoritarian (r = 0.593). Conclusion: All leadership methods are significantly correlated with ICU nurse performance. The transactional style is the most dominant due to clear regulations, SOP evaluation, and reward-punishment systems that provide procedural certainty for staff nurses. It is recommended that head nurses adopt a situational leadership approach by combining transformational motivation and transactional compliance. | 1 |
