| Abstract | Elderly patients with chronic comorbidities are at high risk of experiencing Drug Related Problems (DRPs) due to the use of multiple medications. This study is aimed to identify Drug Related Problems and analyze medication patterns among geriatric patients with chronic comorbidities at RSU Sembiring Deli Tua. This study used a descriptive retrospective design using medical records of patients from May to December 2025. A total of 35 patients were included using the total sampling technique. Drug Related Problems were identified based on the Pharmaceutical Care Network Europe (PCNE) Classification version 9.1. The results showed that most patients were female, with the majority aged 60–70 years. The medication pattern indicated that metformin was the most frequently prescribed antidiabetic drug, while candesartan was the most frequently prescribed antihypertensive drug. The most common Drug Related Problem identified was suboptimal effect of drug treatment, and 11 cases of potential drug interactions were found. Statistical analysis showed no significant association between the number of drugs and the occurrence of drug interactions. There was a tendency for drug interactions to increase with a higher number of medications used. In conclusion, Drug Related Problems were still identified among geriatric patients with chronic comorbidities, indicating the need for regular medication monitoring to optimize treatment outcomes.
