| Abstract | Pharmaceutical services are a crucial subsystem that determines the quality of care and patient safety in hospitals. However, RSUD dr. Djasamen Saragih Pematangsiantar faces the challenge of a high incidence of stock-outs in essential medicines and Medical Consumables (BMHP). This study aims to comprehensively analyze pharmaceutical logistics management using an Input-Process-Output system approach based on Donabedian's quality theory. This qualitative case study involved 13 informants selected through purposive sampling, representing strategic management, functional pharmacy staff, and clinical service units. Data were collected through in-depth interviews with an average duration of 30 to 60 minutes, field observations, and document reviews, which were then analyzed using thematic content analysis. The results in the input dimension revealed that the medicine availability crisis is rooted in weak financial liquidity due to delayed claim disbursements, triggering debt accumulation and supply suspension by distributors. This baseline condition is exacerbated by the technological infrastructure (SIMRS) that is not accurately synchronized with physical warehouse stock. In the process dimension, the supply chain is further hampered by the absence of an early warning system and the highly rigid bureaucracy of external Procurement Officers at the UKPBJ. This series of operational barriers culminates in the output dimension, namely the delay of critical medical therapies and the forced shift of medication financing burdens to patients. It is concluded that medical logistics scarcity is a manifestation of systemic managerial weaknesses. Hospital management is recommended to maximize BLUD flexibility by establishing a pharmacy reserve fund, integrating the inventory information system, and placing Procurement Officers organically within the institution.
