Abstract Infectious diseases caused by Staphylococcus aureus and Escherichia coli remain a common public health problem. Irrational use of antibiotics may lead to bacterial resistance; therefore, alternative antibacterial agents derived from natural products are needed. Vegetable fern (Diplazium esculentum) contains several secondary metabolites, including alkaloids, flavonoids, tannins, saponins, and steroids, which have potential antibacterial activity. This study aimed to determine whether the ethanolic extract of vegetable fern could be formulated into a paper soap preparation, evaluate its antibacterial activity against Staphylococcus aureus and Escherichia coli, and determine the most effective extract concentration. This laboratory experimental study used ethanolic extract prepared by maceration with 96% ethanol and formulated into paper soap containing extract concentrations of 30%, 35%, and 40%. The preparations were evaluated for organoleptic properties, pH, foam height, and washing time. Antibacterial activity was determined using the agar well diffusion method. Data were analyzed using One-Way ANOVA followed by a Post Hoc test. The results showed that all formulations got the required evaluation standards. Statistical analysis demonstrated a significant difference among treatment groups (p = 0.000; p<0.05). The 40% extract concentration exhibited the highest antibacterial activity, producing inhibition zones of 20.26 mm against Staphylococcus aureus and 19.55 mm against Escherichia coli. The conclusion of the study conducted that the ethanolic extract of vegetable fern (Diplazium esculentum) can be successfully formulated into a paper soap preparation, possesses antibacterial activity against Staphylococcus aureus and Escherichia coli, and that the 40% extract concentration is the most effective. Based on the results of this study, it is suggested that future research prepare ethanol extracts of the vegetable fern (Diplazium esculentum) in different dosage forms and test them against bacterial strains other than those used in this study. 1
