| Abstract | Inflammation is a physiological response to tissue injury or infection that may lead to tissue damage when prolonged. Uncaria gambir Roxb. leaves contain catechins, flavonoids, tannins and polyphenols with potential anti-inflammatory properties. This study aimed to formulate the ethanol extract of Uncaria gambir Roxb. leaves into a transdermal patch, evaluate its physical characteristics and determine its anti-inflammatory activity through in vitro and in vivo studies. The ethanol extract was obtained by maceration using 96% ethanol and formulated into transdermal patches containing 5%, 10% and 20% extract using the solvent casting method. Physical evaluations included organoleptic properties, weight uniformity, thickness, pH and stability. In vitro anti-inflammatory activity was evaluated using the Bovine Serum Albumin (BSA) protein denaturation inhibition assay, whereas in vivo activity was assessed using the carrageenan-induced paw edema model in male Wistar rats. The results showed that all formulations met the required physical quality standards for transdermal patches. The ethanol extract exhibited strong anti-inflammatory activity with an ICse value of 71.59 ppm. One-way ANOVA revealed significant differences among treatment groups (p<0.05). Formula III containing 20% extract demonstrated the highest anti-inflammatory activity, with 75.55% inhibition of paw edema at the sixth hour. In conclusion, the ethanol extract of Uncaria gambir Roxb. was successfully formulated into a transdermal patch with acceptable physical characteristics and demonstrated promising anti-inflammatory activity in both in vitro and in vivo studies.
