Abstract Nova Kristiani Waruwu. Formulation and testing of the antibacterial activity of an ethanol extract gel from rosemary leaves (Rosmarinus officinalis L) with a propylene glycol enhancer system against Staphylococcus epidermidis. Undergraduate Thesis, Bachelor of Pharmacy Program, Faculty of Pharmacy, Deli Husada Health Institute, Deli Tua. (Advisors: apt. Pintata Sembiring, S.Farm., M.K.M., M.Farm) Skin infections caused by Staphylococcus epidermidis remain a public health issue requiring the development of alternative therapies based on natural ingredients. Rosemary leaves (Rosmarinus officinalis L.) are known to contain secondary metabolites with potential antibacterial properties. This study is aimed to formulate an ethanol extract of rosemary leaves into gel formulation by using a propylene glycol enhancer system, The experimental research methods included extract preparation via maceration using 96% ethanol, phytochemical screening, and gel formulation with varying concentrations of rosemary leaf extract (25%, 30%, and 35%) and propylene glycol (8.5%, 11.5% and 14.5%). The gel formulations were then evaluated for organoleptic properties, homogeneity, spreadability, pH, viscosity, stability and antibacterial activity using the disk diffusion method. The data were analyzed using a one-way ANOVA. The results showed that ethanol extract of rosemary leaves had yield of 19.34% and contained alkaloids, flavonoids, saponins, tannins and steroids/terpenoids. All formulations met the physical evaluation requirements with a spread of 5.8–6.3 cm, pH of 4.7–5.8 and viscosity of 7316–9316 cP. The formulations were stable when stored at 25°C and 4°C; however, at 40°C, a decrease in pH and viscosity occurred, along with precipitation in Formulas 2 and 3. Antibacterial activity testing showed inhibition zone diameters of 11.73 ± 0.38 mm for formula 1, 13.10 ± 0.17 mm for formula 2 and 16.13 ± 0.12 mm for formula 3, all of them were classified as strong. The results of the one-way ANOVA test showed significant differences among the formulations (p<0.05). Based on the research results, it can be concluded that rosemary leaf ethanol extract can be formulated into a gel preparation that meets physical quality requirements. Variations in propylene glycol concentration affect the physical characteristics of the preparation, and formulation 3 provides the best antibacterial activity against Staphylococcus epidermidis 1
