Abstract Background : Medical records are a crucial component in healthcare services, serving as a comprehensive center for clinical information, a legal basis for evidence, and an instrument for evaluating the quality of patient care. In accordance with the Regulation of the Minister of Health of the Republic of Indonesia Number 24 of 2022, every healthcare provider is obligated to fill out medical records completely, clearly, and accurately immediately after services are rendered. However, field phenomena often indicate a low level of compliance regarding documentation accuracy and completeness, which can directly result in the poor overall quality of medical record documents. Objective : This study aims to analyze and empirically prove the relationship between the completeness of medical record filling and the quality of medical record documents at RSU Cut Meutia, North Aceh Regency. Methods : This research is a quantitative observational study utilizing a cross-sectional design approach. The research sample consisted of 298 medical record documents collected through an observation checklist instrument. The data analysis included univariate analysis to determine the frequency distribution and bivariate analysis using the Spearman Rank non-parametric correlation test. Results : The univariate analysis revealed that the majority of medical record fillings were in the "Less Complete" category, comprising 229 documents (76.8%), followed by the "Incomplete" category (19.5%), and only a small fraction met the "Very Complete" criteria (3.7%). Correspondingly, the quality of medical record documents was heavily dominated by the "Poor Quality" category with 230 documents (77.2%), followed by "Very Poor Quality" (19.1%), and "Good Quality" (3.7%). The Spearman Rank bivariate statistical test proved a highly significant relationship (p-value = 0.001 < 0.05). The correlation coefficient obtained was very strong (r = 0.988) with a positive (unidirectional) relationship. Conclusion : There is a significant, very strong, and positive relationship between the completeness of medical record filling and the quality of medical record documents at RSU Cut Meutia, North Aceh Regency. The more completely a medical record document is filled out, the better the quality of the document will be. The hospital management is highly advised to tighten supervision Evaluating Incomplete Medical Record Filling and promptly optimize the implementation of an integrated Electronic Medical Record system.
