Abstract ABSTRACT Health insurance for children is crucial, as childhood is a vital period of growth and development that determines their current health status. This study aims to examine the relationship between the use of health insurance services and access to health facilities regarding the nutritional status of children in East Halongonan District, North Padang Lawas Regency. This study employed an analytical survey design (explanatory research) using a cross-sectional approach, where data on both dependent and independent variables were collected simultaneously. Primary data were obtained through questionnaire-guided interviews with mothers of children aged 0–5 years who consented to participate; children's nutritional status was assessed using WHO and CDC growth curves. The results indicate no significant relationship between health insurance ownership and children's nutritional status (p > 0.05). The Prevalence Ratio (PR) was 0.86 for those with insurance and 3.61 for those without; while the PR suggests a higher risk of malnutrition among the uninsured group (3.61 times higher), the p-value of 0.08 indicates no statistically significant association between health insurance coverage and the nutritional status of children under five. Conversely, a significant relationship was found between access to health facilities and children's nutritional status in East Halongonan District (p < 0.05). The PR was 1.28 for easily accessible facilities and 0.07 for those with difficult access, indicating that the group with easy access had a malnutrition risk 0.07 times that of the group with difficult access. In conclusion, access to health facilities is significantly associated with children's nutritional status, whereas health insurance ownership is not significantly associated with children's nutritional status in East Halongonan District, North Padang Lawas Regency.
