| Abstract | Children's health must be safeguarded through the administration of a complete series of basic immunizations. This study aimed to analyze the factors influencing low basic immunization coverage in the working area of the Longat Community Health Center (Puskesmas), Mandailing Natal Regency, in 2026. An observational analytic study design with a cross-sectional approach was employed. The sample consisted of 70 mothers with children aged 12–24 months, selected using accidental sampling. Independent variables included maternal education, employment status, age, distance to health services, knowledge, the role of health workers, and family support. The dependent variable was the completion of the basic immunization series, based on the child's immunization status. Data analysis involved univariate analysis, the chi-square test, and multiple logistic regression. Chi-square test results indicated that maternal education (p=0.021), maternal age (p=0.008), distance to health services (p=0.002), maternal knowledge (p=0.031), and family support (p=0.035) were significantly associated with the administration of basic immunizations. Maternal employment status (p=0.223) and the role of health workers (p=0.831) showed no significant association. Logistic regression results identified family support as the dominant factor (OR 11.235; p=0.007). The study demonstrates that increasing basic immunization coverage requires the involvement of the family—particularly husbands and family members who play a role in decision-making regarding children's health.
