Abstract The Free Health Check-up Service (Pemeriksaan Kesehatan Gratis/PKG) is a primary healthcare policy designed to improve access to preventive screening and early disease detection at primary healthcare centers (Puskesmas). However, the implementation of this policy at Ujung Batu I Primary Health Center, Padang Lawas Regency, has not yet been optimal, as reflected by a low service coverage rate of only 402 out of 1,539 target individuals (26.13%). This gap indicates a discrepancy between policy objectives and actual community utilization, necessitating an in-depth analysis of policy implementation. This study aims to analyze the implementation of the Free Health Check-up Service policy at Ujung Batu I Primary Health Center in 2025 using the policy implementation framework of Van Meter and Van Horn, which includes six dimensions: policy standards and objectives, resources, inter-organizational communication, characteristics of implementing agents, implementer disposition, and socio-cultural and economic environment. A qualitative research approach with a case study design was employed. Informants included the head of the health center, healthcare providers, program officers, and community members. Data were collected through in-depth interviews, observation, and document review, and analyzed using thematic analysis through data reduction, data display, and conclusion drawing with triangulation. The findings reveal that policy standards are formally established through standard operating procedures and technical guidelines; however, public understanding remains limited, as the program is still perceived primarily as curative free treatment rather than preventive screening. Human resources and health infrastructure remain insufficient, leading to task overlapping among staff. Communication and coordination have not yet effectively reached all service areas. Although the organizational structure is well defined, high workload affects implementation performance. Implementers generally demonstrate a positive disposition, but operational constraints reduce effectiveness. Socio-cultural factors represent the main barrier, particularly low health-seeking behavior for preventive care. In conclusion, the implementation of the policy remains suboptimal, particularly in communication, resource availability, and community acceptance. Recommended improvements include strengthening community-based health education, optimizing the role of community health workers, enhancing data integration systems, improving human resource allocation, and reinforcing inter-sectoral coordination.
