| Abstract | This study aimed to analyze the implementation of the electronic medical record (EMR) system at the outpatient registration unit of UPT Puskesmas Helvetia, Medan City, in 2026 using the PIECES framework. A descriptive qualitative design was employed, involving three informants selected through purposive sampling. Data were collected through interviews, observation, and documentation, and analyzed using the Miles and Huberman model. Data validity was established through data and source triangulation. The findings showed that the performance aspect improved through faster data retrieval and patient registration. The information aspect indicated that patient data were generally accurate, complete, and updated in real time, although input errors and duplication remained possible. The economic aspect demonstrated reduced paper and office stationery use, while maintenance costs remained a challenge. The control aspect included access restrictions and administrator monitoring, although shared-account use had occurred. The efficiency aspect showed simplified administrative tasks but remained constrained by internet connectivity, BPJS server disruptions, and vendor-related issues. The service aspect indicated faster registration and generally positive patient responses, although system disruptions and limited facilities remained obstacles. Overall, EMR implementation supported the effectiveness, efficiency, and quality of outpatient registration services. Improvements in infrastructure, system stability, maintenance, and user competence are still needed. | 1 |
