| Abstract | Novianti, 2026. Factors Associated with the Completeness of Files and Forms in the Implementation of Electronic Medical Records (EMR) at RSUD dr. Fauziah Bireuen. Undergraduate Thesis, Health Information Management Study Program, Deli Husada Health Institute Deli Tua. This study was prompted by ongoing implementation issues regarding Electronic Medical Records (EMR). Pursuant to the Regulation of the Minister of Health Number 24 of 2022, complete documentation reaching 100% is required to guarantee service quality, patient safety, legal protection, and seamless health financing claim processes. However, objective conditions at RSUD dr. Fauziah Bireuen indicate that incomplete entries in EMR files and forms still occur across several inpatient units. This study aimed to analyze individual factors, organizational factors, and technological factors associated with the completeness of files and forms in EMR implementation at RSUD dr. Fauziah Bireuen. This study employed a quantitative method with a cross-sectional design. Data were collected using questionnaires distributed to respondents and observation sheets. The sample comprised 89 medical personnel and healthcare professionals involved in electronic medical record entry at RSUD dr. Fauziah Bireuen. Hypotheses were formulated and tested using univariate analysis to describe the characteristics of each research variable, and bivariate analysis to examine variables presumed to be correlated. The results showed that among 89 EMR files, 53 files (59.6%) were categorized as complete and 36 files (40.4%) were incomplete. Analysis revealed no significant association between individual factors or organizational factors and EMR completeness. Conversely, there was a statistically significant association between technological factors and the completeness of EMR files and forms (p = 0.008; PR = 1.982). Technological factors represent the primary determinant significantly associated with EMR file and form completeness at RSUD dr. Fauziah Bireuen. It is recommended to optimize the application interface (UI/UX), provide computer hardware, strengthen network infrastructure, and implement reminder features (mandatory fields) to prevent incomplete medical data. | 1 |
