Abstract Delayed referral is a cause of maternal mortality. Delay in seeking care is a delay that occurs at the community level. It can be caused by individuals, health facilities and the environment. This study aims to identify factors that influence delay in seeking care nationally in Indonesia. The research design was an ecological study using 2018 Basic Health Research data with a unit of analysis of 514 districts/cities in Indonesia. Univariate analysis in the form of area mapping and bivariate using Pearson or Spearmen correlation test. The variation in referral delays between districts/cities in Indonesia was wide, ranging from 6.9% to 100%. The bivariate test results proved that education level, parity, ANC visits, ANC place, rural area, access to health facilities had an effect on referral delays. However, employment status and national health insurance (JKN) participants have no effect on referral delays. It is expected that the government can increase intensive education for all pregnant women and the community about danger signs and preparation for childbirth to deal with complications, as well as improving the quality of services at Public Health Centre and equalising the availability of Health Facilities to be easily accessible to people in rural and remote areas. id
